Secure, on-line processing for new and renewing members who would like to remit their dues using a credit card.


If you are a new member or one who is rejoining, please fill out all data fields.


Renewing members should fill out required fields and any data that was missing from your personal invoice or has changed.

Please complete the membership information form then scroll down for pricing.

To remit dues using a credit card, do not click "submit". Scroll to the bottom of the page and click "add to cart" to complete your order on the shopping cart page. When on the shopping cart page, you can select the correct membership level
.


If you would like to pay by check,  click "submit". Mail your check payable to CSHP, to CSHP c/o The Association Advantage LLC, 591 North Avenue, Ste. 3-2, Wakefield, MA 01880


Please note:
Membership is official when both a completed form and dues are received.

Membership Levels:

**For membership beginning immediately and running through Dec. 31, 2016


  • Active/Pharmacist - $100**
  • ActiveJoint (Married Partners) - $150 ($75.00 each)** Please remit two applications and one payment
  • Residents, Fellows - $40**
    Retired Pharmacists- $35 **
  • Pharmacy Technician-$35 **
  • Associate (other than working pharmacist or technician) - $125**
  • Student members - no charge but please be sure to submit a student application
